The National Counselor exam is a pencil and paper formatted test consisting of 200 multiple-choice questions. Examinees are given three hours and forty-five minutes to complete the exam. The questions are curated from the original eight content areas established by the Council for Accreditation of Counseling & Related Educational Programs (CACREP) and five work behaviors derived from NBCC's national job analysis. These questions have undergone rigorous review and field-testing, ensuring their reliability and validity.

Sitting for the NCE is essential for individuals who aspire to become National Certified Counselors (NCC) or seek counselor licensure in many states. Additionally, military health providers must pass the NCE as a pre-condition to becoming authorized counselors within the military healthcare system.
